Declarer leads the ♦7.
This places West in the dilemma of "Morton's Fork" !
If West plays the ♦A (winning the trick), Declarer's ♦K will be a winner, and Dummy's ♦Q and ♣A can be used to discard the two losing hearts in Declarer's hand.

If, on the other hand, West plays low, Dummy's ♦Q wins the trick. Dummy's ♣A is then lead on which Declarer discards his ♦K.
The Opponents now will only be able to win one trick in .
The critical point is really trick 1, where the ♣A is not played immediately, so it can later be used for a discard of either a or a in Declarer's hand.
By ruffing the first trick instead, both these discard options are kept alive !
